Patient Assessment Coordinator - RN / LPN / Social Worker

CommuniCare Health Services is currently recruiting a Patient Assessment Coordinator to identify patients for potential admission into our Akron/Youngstown, OH area facilities and to maintain positive relationships with current referral sources. The Patient Assessment Coordinator will facilitate admissions by coordinating transmission of clinical and benefit information from referral sources back to the SNF admissions teams, administrations, and third party payers.
The Payment Assessment Coordinator will preferably have an RN or LPN license, a degree in Social Work, or other medical credentials.
Essential responsibilities include:
ThePatient Assessment Coordinator achieves these objectives by providing excellent, responsive customer service and accurate, timely, and comprehensive clinical assessments and by proactively working with all departments in the facility.
Qualified candidates for this position will have an RN or LPN license in the State of Ohio, a Social Work degree, and/or other medical credentials. The ideal candidate will preferably have three years' treatment experience in a Long Term Care setting. Previous clinical screening experience preferred, along with recent healthcare experience in an Long Term Care facility. Candidates will also preferably have knowledge of the local medical community and an outgoing, energetic personality .
